1095 Documents Available Upon Request
Due to recent legislation under the Paperwork Burden Reduction Act, QAI will no longer automatically provide printed 1095 forms to employees. In prior years, you may have received these forms without making a request. However, under the updated law, 1095 forms are now available only upon request. If you or your tax preparer requires a copy, please complete the form below (if you can not see the form email hr@qai.org).
