    The following form allows QAI clients to order Fire Door or Fire Door Frame Labels.

    Please fill in the following information in order to submit your order. A QAI representative will receive and start processing your order upon receipt.

    Once submitted you will receive an email to the specified email address.

    If you have any corrections to your order, or label type required not listed, please contact us as as soon as possible at +1(604)527-8378 or labels@qai.org.

    Notes: Minimum Order Quantity = 500 units per label type.
    Prices are in U.S. dollars. Conversion rate will be applied for orders in Canadian Dollars.
    Please note our updated pricing for 2024:

    • $1.20 per regular Mylar Label
    • $1.25 per regular Metal Tag — 1 color
    • $1.67 per regular Metal Tag — 2 color

    Special order requests (such as premask, barcodes, nailing holes, adhesive backing, etc.) incur additional charges per label. Please contact us to inquire about special order pricing.
    Additional setup fee will be charged when applicable.
    Normal delivery time is 2 weeks after order receipt + shipping time. If order is required sooner, rush order is required for which a rush fee will be applied.
    Prices are subject to change based on future increases from our suppliers.
